Job summary

University of Florida Jacksonville Physicians, Inc. seeks an Advanced Practice Provider I to deliver direct patient care in primary outpatient settings across community health, family medicine, and pediatrics.

Licensure in Florida and/or Georgia required; supervision by a physician. Responsibilities include conducting history and exams, developing health plans with the care team, prescribing and coordinating care, and ensuring proper documentation in EMR systems.

Qualifications

• 2 years outpatient setting preferred.
• High School Diploma or GED is required.
• Master's Nurse Practitioner or Master's Physician Assistant program required.
• BLS certification required at time of hire.
• Advanced Practice Nurse Practitioner License required at time of hire.
• Physician Assistant License required at time of hire.

Responsibilities

• Obtains patient histories, performs exam and tests, and orders/ interprets diagnostic tests.
• Formulates health management plans with physicians and other professionals.
• Reviews, prescribes and/or recommends medications and treatments.
• Provides timely referrals to other medical services as needed.
• Educates and counsels patients and families on health care.
• Documents encounters and completes EMR entries per guidelines.
• May provide education and instruction to patients.
• Travel up to 25% and provide services across UF Health facilities.
