The APP will collaborate with gastroenterology physicians to manage acute and chronic GI conditions, coordinate procedures, and ensure safe transitions of care while providing high-quality, patient-centered care. 7-day coverage in a 14-day cycle, alternating weekends and holidays, is required.

Qualifications

Full-Time Advanced Practice Provider - Inpatient Gastroenterology

Aultman Hospital – Canton, Ohio

Position Highlights

• Full-time, hospital-employed position within the Aultman Health System
• Collaborates to provide comprehensive inpatient gastroenterology consultation, evaluation, treatment, management and follow-up for hospitalized patients with acute and chronic gastrointestinal and hepatobiliary conditions.
• Develops and implements evidenced-based plans of care; procedural interventions; coordinate procedural planning with the gastroenterology physicians; support safe transitions of care and promote efficient, high quality inpatient GI services.
• 7-day coverage in a 14-day period
• Includes every other weekend and every other holiday

Position Responsibilities

• Maintains the standards of the Inpatient Gastroenterology Service and adheres to established gastroenterology clinical care expectations.
• Evaluates and manages common inpatient gastrointestinal and hepatobiliary conditions, including but not limited to gastrointestinal bleeding, anemia with suspected GI source, abdominal pain, nausea/vomiting, dysphagia, diarrhea, constipation, inflammatory bowel disease, pancreatitis, abnormal liver tests, cirrhosis and its complications, and other acute GI disorders.
• Collaborates with gastroenterology physicians regarding patients requiring endoscopic procedures, advanced diagnostic evaluation, procedural intervention, transfer to a higher level of care, or physician-level escalation.
• Performs new inpatient gastroenterology consultations and subsequent follow-up visits, including comprehensive history and physical assessment, review of relevant laboratory and imaging data, development of differential diagnoses, and formulation of treatment plans within scope of practice.
• Identifies clinical urgency and prioritizes consultations based on acuity, including recognition and prompt escalation of hemodynamic instability, active gastrointestinal bleeding, acute liver failure, cholangitis, severe pancreatitis, or other time-sensitive GI conditions.
• Orders, interprets, and follows appropriate diagnostic studies and therapeutic interventions in accordance with scope of practice, clinical protocols, and collaborating physician guidance.
• Coordinates preparation and clinical readiness for endoscopic and other GI-related procedures, including review of anticoagulation/antiplatelet therapy, diet status, bowel preparation, pertinent laboratory testing, and communication of procedural plans to the primary team and patient.
• Provides patient and family education regarding gastrointestinal diagnoses, recommended testing or procedures, treatment plans, medications, diet, follow-up needs, and warning signs requiring reassessment.
• The APP does not independently perform endoscopic procedures unless separately credentialed and privileged to do so; procedural indications, timing, and performance are coordinated with the gastroenterology physician.
• Communicates recommendations clearly and promptly with the primary hospital team and other consultants and documents the gastroenterology plan of care in the medical record.
• Participates in multidisciplinary care coordination to reduce delays in diagnostic evaluation, procedures, discharge, and outpatient GI follow-up.
• Supports appropriate transitions of care by identifying outpatient gastroenterology follow-up needs and communicating pending studies, pathology, procedure results, and recommended surveillance when applicable.
• If needed, participates in the orientation, education, and clinical development of new APPs and other personnel assigned to the Inpatient Gastroenterology Service.
• Follows the policies, standards, protocols, and procedures of the hospital and the Division of Nursing Service.
• Demonstrates the ability to provide individualized, goal-directed care through advanced assessment, clinical reasoning, evidence-based decision-making, and ongoing reassessment of patient response to treatment.
• Communicates and collaborates with gastroenterology physicians and members of the interdisciplinary healthcare team in a timely and professional manner.
• Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation.
• Supports strategic organizational goals, including timely access to inpatient gastroenterology consultation, efficient throughput, appropriate resource utilization, and continuity between inpatient and outpatient GI care.
• Assess the needs of the patient for discharge planning.
• Provides patients / families education and support.
• Assists physicians with tests and procedures.
• Participates in economical utilization of supplies.
• Reports and accurately records pertinent observations and reactions regarding patients to the appropriate person (ie : physicians, unit director, division director, charge nurse).
• Develops and leads team conference.
• Participates in the orientation of new personnel.
• Demonstrates an ability to competently provide individualized, goal-directed nursing care to patients through use of assessment.

• Maintain current ACLS certification and respond per AIM policy to medical emergencies while in the facility.
• Complete all Medical Records as required by Medical Staff bylaws, Rules and Regulations of Hospital and per AIM policy.
• Comply with established record keeping, billing and coding per AIM policy

Qualifications

• Licensed to practice as either an Advanced Nurse Practitioner or Physician Assistant in the State of Ohio
• Minimum one year of acute care experience preferred. Inpatient gastroenterology, hepatology, hospital medicine, critical care, or related specialty experience strongly preferred.
• Excellent interpersonal and oral/written communication skills; strong organization skills; ability to stimulate new ideas and create an environment of teamwork
