Job summary

2nd Chance Clinic in Somerset, KY is seeking an experienced Advanced Practice Provider (NP/PA) to join our addiction treatment team. The role emphasizes prevention, screening, intervention, and treatment for substance use disorders, with a focus on ethical and patient-centered care.

A background check is required; initial schedule is one day per week with potential to expand to five days as the practice grows.

Qualifications

• Kentucky license as NP or PA (KBML or KBN) in good standing.
• DEA registration current and active.
• Current BLS or ACLS certification.
• CAQH profile up to date.

Responsibilities

• Provide prevention, screening, intervention and treatment for substance use and addiction.
• Apply ethical, evidence-based practices in patient care.
• Build and maintain a patient-focused addiction treatment program.
