Qualifications

• Active NP or PA license required or in process.

• Strong communication and clinical skills; valid driver’s license with reliable transportation.

Responsibilities

• Provide mobile primary care visits to seniors in homes and facilities.
• Manage chronic and acute conditions with comprehensive assessments.
• Develop care plans and coordinate with caregivers and interdisciplinary teams.
• Deliver patient-centered care with accurate EMR documentation.
• Collaborate with clinical leadership and nursing/therapy teams.

Western Geriatrics and Neurology, part of Brigade Health, is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes, skilled nursing facilities, assisted living, and senior communities.
