Job summary

Sentara Health is seeking an experienced APP to join the Community Care program in South Boston, VA. You will co-manage patients with a multidisciplinary team, focusing on family medicine, pediatrics, and adult complex cases, with care coordinated across RN, counselors, and care management.

The role includes clinic and event coverage about 1 day per week, review of outcomes with the care team, and collaboration with local leaders on community education, supporting Sentara's mission and expanding

Qualifications

• 1+ year APP experience required.
• Experience in FQHC or Community Health Center would be beneficial.
• Treating family medicine, pediatrics, and adults with complex issues.

Responsibilities

• Review quality and outcome metrics monthly with the healthcare team.
• Collaborate with local civic and faith-based leaders on community education programs.

• Current needs include clinic and event coverage averaging about 1 day per week
• Treating family medicine, pediatrics, and adults with complex issues
• Shared call schedule, with RN first call
• Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
• Review quality and outcome metrics monthly with the healthcare team
• Collaborate with local civic and faith-based leaders on community education programs
• Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
