The Advanced Practice Provider (APP), CareLine, PRN is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. Providing on-call coverage, the incumbent serves as the dispositional authority for after-hours and weekend clinical calls. He/She assesses, diagnoses, treats, and documents geriatric patients’ physical and psychosocial health status through data collection.

Essential Job Duties/Responsibilities
  • This is a PRN/Per Diem position, requiring a minimum of 160 hours per year, with scheduling flexibility based on department needs.
  • Works on an as-needed basis, including nights, weekends, and holidays.
  • Expected to pick up shifts to cover PTO, LOA, and other absences, with some shifts planned in advance and others requiring minimal notice.
  • Assess acute and non-acute clinical problems via virtual video conference or telephone. Perform and document physical evaluations and patient histories, analyze trends, and develop management plans based on findings. Aid in care plans including health education, referrals, and counseling.
  • Evaluate need for immediate intervention, consultation, or referral, and facilitate necessary care.
  • Plan patient care considering cultural background, understanding, personality, and support systems. Serve as patient advocate.
  • Collect and analyze comprehensive health data to determine maintenance and improvement strategies.
  • Coordinate with PCPs and other providers to report data and ensure guideline compliance.
  • Consult with patients and families on health outcomes and promote positive health habits.
  • Lead efforts to ensure optimal outcomes through Telemedicine, collaborating with on-call PCPs and implementing protocols.
  • Document assessments, interventions, and progress clearly and accurately.
  • Enhance telehealth practices through data utilization and system improvements.
  • Communicate effectively across various formats and tools, building professional relationships.
  • Utilize resources to provide safe, effective, and responsible services.
  • Deliver exceptional customer service and professionalism.
  • Participate in clinical rounds, meetings, and quality programs as appropriate.
  • Adhere to departmental standards, policies, and practice agreements.
  • Contribute to the development of telehealth policies and procedures.
  • Perform other duties as assigned at the manager’s discretion.
Knowledge, Skills, and Abilities
  • Expert-level business acuity and understanding of core job functions.
  • Knowledge of medical practices for independent and collaborative practice.
  • Proven performance in clinical metrics.
  • Technical proficiency in telemedicine per regulations.
  • Excellent clinical judgment, problem-solving, and prioritization skills.
  • Effective communication and collaboration abilities.
  • Willingness to travel within South Florida (~10%) and work flexible hours, including evenings, overnights, weekends, and holidays.
  • Proficiency in Microsoft Office Suite; bilingual skills (Spanish/Creole) a plus.
  • Ability to exercise independent judgment and maintain expertise.
Education and Experience Requirements
  • Bachelor's in Nursing (BSN) and graduate of an accredited nursing program with certification in specialty; Master's degree required.
  • Active RN license required; Board certification (AANP or ANCC) for NPs.
  • BLS certification from AHA or Red Cross within 90 days of employment.
  • DEA registration if prescribing controlled substances.
  • Multi-state licensure including FL, VA, and at least two other states from the specified list.
  • Minimum 3 years of clinical experience in acute/primary care; 2 years telehealth experience preferred.
