Endeavor Health Immediate Care is seeking experienced APN/PA to join a fast-paced team in northern Illinois. You will work under collaborating physicians to provide outpatient care, perform exams, order tests, interpret results, and prescribe appropriate therapies.

The role emphasizes autonomous practice within an interdisciplinary team, commitment to evidence-based care, and contributing to high-quality patient experiences across multiple clinics in Cook and Lake Counties.

Qualifications

• License: Advanced Practice Nurse Licensure (APN) or Physician Assistant (PA) licensure in IL.
• Master’s level program in specialty area (MSN/MS or NCCPA certified PA).
• BLS/CPR for Healthcare Provider certification required.
• DEA and Illinois Controlled Substance License required for the role.
• Minimum 2 years Emergency Department or IC experience as APN/PA.

Responsibilities

• Under collaborating physicians, provides care in outpatient immediate care.
• Obtains history, performs physical examinations, orders imaging and labs.
• Interprets results and instructs on therapies.
• Maintains records and writes progress notes; follows patients through treatment.
• Collaborates with physicians and health care team to achieve outcomes.

Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen. Maintains records and writes progress notes on status, treatments, and procedures. Follows patients through their course of treatment in the immediate care. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Works autonomously and in interdisciplinary teams. Positively contributes to safety outcomes and promotes high quality patient experiences. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization.
