Qualifications

What you can expect in this role:

· Provide comprehensive care across multiple settings-- in-office visits and telehealth.

· Ability to diagnose routine conditions and consult (or refer to) physicians for more complex diagnoses.

· Treat patients by following best practice protocols and consult with physician leaders if you deem it necessary to modify the care pathways.

· Communicate with your patients as necessary to ensure we offer a delightful experience.

· Manage patient treatment plan and provide support and education to patients on the way.

· Educate patients about Ascend’s broader offering, conduct health risk assessments and guide them on their journey to longevity and happiness.

· Leverage cutting-edge technology to streamline workflows, reduce administrative burdens, and focus on delivering high-quality care

Requirements

Requirements

· Master’s degree from an AMA accredited school for Advanced Practice Providers.

· Current, active PA or NP licensure in State of employment is required.

· Internal Medicine or Family Medicine Board Certification (or board eligible)

· Active, non-probationary, unrestricted Georgia State License

· Active DEA license or eligibly to obtain

· 1+ year clinical experience in internal medicine or primary care practice setting.

· Technological proficiency—comfortable using EMRs and digital health tools to enhance care delivery
