Griffin Health is seeking an experienced Advanced Practice Provider to join our Internal Medicine/Family Medicine team. The ideal candidate will have at least five years of clinical experience providing comprehensive, patient-centered care to adult and/or family medicine populations.

• National board certification appropriate to the provider's discipline.
• Minimum of 5 years of clinical experience in Internal Medicine, Family Medicine, Primary Care, or related setting.
• Current BLS certification; additional certifications as required.

Responsibilities

• Provide comprehensive assessment, diagnosis, treatment, and follow-up care for patients in an Internal Medicine/Family Medicine setting.
• Perform histories and physical examinations and develop individualized treatment plans.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory and diagnostic studies.
• Prescribe medications and treatments within the scope of practice.
• Provide preventive care, health education, and counseling.
• Manage chronic disease conditions and coordinate referrals.
• Maintain accurate clinical documentation in the electronic medical record.
• Collaborate closely with physicians, nurses, specialists, and other team members.
• Promote evidence-based medicine, patient safety, and high-quality outcomes.
• Participate in quality improvement initiatives and patient-centered care activities.

This is an excellent opportunity for an experiencedNurse Practitioner (NP) or Physician Assistant (PA)who is committed to delivering high-quality primary care, developing long-term relationships with patients, and working collaboratively within a multidisciplinary healthcare environment.

Key Responsibilities

• Provide comprehensive assessment, diagnosis, treatment, and follow-up care for patients in an Internal Medicine/Family Medicine setting.


• Perform thorough histories and physical examinations and develop individualized treatment plans.


• Diagnose and manage acute and chronic medical conditions.


• Order and interpret appropriate laboratory and diagnostic studies.


• Prescribe medications and treatments within the scope of practice.


• Provide preventive care, health education, and counseling.


• Manage chronic disease conditions and coordinate appropriate specialty referrals.


• Maintain accurate and timely clinical documentation in the electronic medical record.


• Collaborate closely with physicians, nurses, specialists, and other members of the healthcare team.
