• APRN or PA-C degree from an accredited program.
• Minimum of 3 years of experience in hematology/anticoagulation management or thrombosis care.
• Experience in cardiology or vascular medicine preferred.
• Active, unobstructed Nevada and Utah medical licenses, or the ability to obtain one.
• Full prescriptive authority and an active DEA license, or ability to obtain one.
• Ability to complete Intermountain Health’s credentialing process.

In this role, you will provide comprehensive outpatient management of patients requiring anticoagulation therapy and thrombosis-related care in collaboration with a dedicated pharmacist.
