Job summary

LanceSoft Inc. seeks an experienced Nurse Practitioner or Physician Assistant to provide in-home primary care for medically complex, homebound patients in Brooklyn/Staten Island, Bronx/Westchester, NY. You will work with an interdisciplinary team to improve outcomes and reduce hospitalizations.

The role requires NP/PA licensure in NY, active DEA, and 3+ years of clinical experience, with EMR/EHR proficiency and home‑based care experience preferred.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Active New York State NP or PA license and National Board Certification.
• Active DEA license.

Responsibilities

• Provide comprehensive primary care to homebound and high- needs patients.
• Perform assessments, diagnose conditions, and develop individualized care plans.
• Manage chronic diseases, preventive care, and acute medical needs.
• Order and interpret diagnostic tests and prescribe appropriate treatments.
• Document care accurately in the EHR and maintain quality standards.
• Collaborate with physicians, specialists, nurses, and care managers to coordinate care.
• Perform procedures such as wound care, catheter management, and phlebotomy/IV access as needed.
• Participate in case conferences and share after-hours call coverage on a rotating basis.

Compensation: Up to $220,000 total compensation (Base + Performance Incentive) plus $10,000 retention bonus after one year
