Job summary

Local MD Urgent Care in Columbus, IN seeks an Advanced Practice Provider (APP) to provide comprehensive evaluation, diagnosis, and management of patient health needs. The APP will work under medical direction with emphasis on high-quality care and collaborative teamwork.

Responsibilities include patient screening, diagnosis, treatment, education, and accurate EMR documentation. Requires active Indiana APRN or PA license, RN license (NPs only), national certification, BLS, and DOT/FMCSA Medical

Qualifications

  • Active Indiana APRN license or Indiana PA license.
  • Active RN license (NPs only).
  • National Certification (AANP/ANCC for NPs; NCCPA for PAs).
  • BLS certification.
  • DOT/FMCSA Certified Medical Examiner.

Responsibilities

  • Perform initial patient assessments with history-taking and examinations.
  • Diagnose and treat acute and chronic conditions; manage care plans.
  • Prescribe medications with safety and cost considerations.
  • Educate patients and families on self-management.
  • Document thoroughly in the EMR per Local MD standards.
  • Collaborate with Medical Director and clinic team; provide mentorship.

Advanced Practice Provider (APP) – Full‑Time / Part‑Time / Per Diem

Job Description

The Advanced Practice Provider (APP), which includes Nurse Practitioners and Physician Assistants, is an essential member of the Local MD Urgent Care team. The APP is responsible for setting and upholding high standards of care quality and service excellence. Through consultation and collaboration with the Medical Director, the APP provides comprehensive evaluation, diagnosis, treatment, and management of patient health needs. The APP utilizes independent clinical judgment, professional communication, and collaborative teamwork to deliver high‑quality care across the continuum. The role encompasses clinical leadership, patient education, partnership with staff, and ongoing contributions to quality improvement and clinical research. APPs build and maintain effective relationships with patients, families, physicians, and staff to achieve optimal outcomes.

Essential Job Functions & Competencies
Patient Screening & Analysis
  • Performs initial patient assessments, including history‑taking and physical examinations.
  • Establishes baseline health profiles and analyzes diagnostic data.
  • Determines need for follow‑up appointments, additional testing, or changes in treatment plans.
Diagnosis & Treatment
  • Diagnoses and treats acute and chronic health conditions including illnesses, infections, and injuries.
  • Prescribes medications based on efficacy, safety, cost considerations, and legal authorization.
  • Manages patient care plans and coordinates ongoing treatment needs.
Professional Care Service Recommendations
  • Recommends appropriate on‑site and off‑site medical services within scope of training and specialty.
  • Coordinates referrals and collaborates with specialty providers as needed.
Patient Education
  • Educates patients, family members, and guardians on self‑management of acute and chronic illnesses.
  • Tailors education and instructions to each patient’s unique circumstances and level of understanding.
Patient Records
  • Prepares accurate, thorough, and timely documentation in the EMR per Local MD standards.
  • Uses the EMR to complete documentation, medication orders, diagnostic orders, and referrals.
Education & Training
  • Provides medical direction, education, and training to clinical team members involved in patient care.
  • Demonstrates leadership through mentoring, communication, and modeling best practices.
Organization‑Specific Expectations

  • Functions effectively as a member of the provider team and collaborates with all clinic staff.

  • Adheres strictly to Local MD charting standards and all clinical/administrative processes.
  • Maintains competency in office procedures and use of medical equipment, computers, and systems.
  • Preserves patient and employee confidentiality at all times.
  • Willingness to travel between clinic locations within a 25‑mile radius.
  • Performs other duties as assigned.
Certification, Licensure, and Registry Requirements
Required for All APPs
  • Active Indiana APRN License (NPs) OR Indiana PA License
  • Active RN License (NPs only)
  • National Certification (AANP/ANCC for NPs; NCCPA for PAs)
  • Basic Life Support (BLS)
  • DOT/FMCSA Certified Medical Examiner
Preferred Certifications
  • Advanced Cardiac Life Support (ACLS)
  • Pediatric Advanced Life Support (PALS)
  • Indiana Controlled Substance License
  • DEA Registration
Educational Requirements
  • Master’s Degree or Doctor of Nursing Practice (NP track) OR Graduation from accredited PA program
