Job summary

Blueprint Healthcare Medical Group seeks a full-time Advanced Practice Provider (NP or PA) to deliver specialized pain management care across partnered skilled nursing facilities in MA.

Pain management training will be provided; this field-based role offers daytime hours, Monday–Friday, with no overnight call and flexibility based on territory as the program scales.

Qualifications

  • Licensed NP or PA with current, active license in applicable state(s).
  • Minimum of 2 years of practice in any specialty.
  • DEA license or eligibility required.
  • Experience in geriatrics, internal medicine, SNF, or post-acute care is preferred.

Responsibilities

  • Perform initial pain evaluations and ongoing management visits in SNF/post-acute settings.
  • Develop and implement multimodal pain management plans (non-opioid, opioid stewardship, adjuvant therapies).
  • Conduct medication optimization, taper plans, and risk mitigation strategies.
  • Collaborate with facility staff, primary care providers, psychiatry/behavioral health teams.
  • Document encounters using compliant, audit-ready templates.
  • Educate facility staff on pain management best practices.

Position Overview

We are seeking a full-time Advanced Practice Provider (NP or PA) to deliver specialized pain management care across partnered skilled nursing facilities.

Pain Management training will be provided.

This is a field-based role ideal for clinicians who want:

Key Responsibilities

  • Perform initial pain evaluations and ongoing management visits in SNF/post-acute settings
  • Develop and implement multimodal pain management plans (non-opioid, opioid stewardship, adjuvant therapies)
  • Perform medication optimization, taper plans, and risk mitigation strategies
  • Collaborate with:
    • Facility staff (nursing, rehab, administration)
    • Primary care providers
    • Psychiatry and behavioral health teams (when appropriate)
  • Document encounters in a timely manner using compliant, audit-ready templates
  • Participate in care coordination and provide education to facility staff on pain management best practices

Clinical Model

  • Moderate visit frequency model focused on appropriate follow-up cadence
  • Emphasis on:
    • Functional outcomes
    • Safe prescribing
    • Regulatory compliance (Medicare, MA plans, SNF regulatory environment)
  • Structured clinical workflows and documentation support

Schedule

  • Full-time (Monday–Friday)
  • Daytime hours
  • No overnight call
  • Optional flexibility based on territory

Qualifications

Required:

  • Licensed Nurse Practitioner (NP) or Physician Associate (PA)
  • Minimal 2 years of practice in any specialty
  • Active and unrestricted license in applicable state(s)
  • DEA license (or eligibility)

Preferred:

  • Experience in geriatrics, internal medicine, SNF, or post-acute care
  • Experience in pain management or palliative care
  • Familiarity with polypharmacy and complex chronic disease management
  • Strong documentation and compliance awareness
