This position has coordination and clinical skills required for the comprehensive management of critical care medicine patients, including biological, social, and psychological assessments. They will possess specialized skills in the care of patients and families, initial & ongoing medical management, and invasive procedures. The incumbent coordinates involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient and reviews the plan of care with the other members of the clinical team, participating in directing that care. The provider will work with the team to ensure adherence to protocol requirements and regulatory guidelines. They will perform diagnostic/therapeutic procedures, peri-procedural services, and coordinate discharge planning to the next level of care (as applicable). The incumbent will participate in the education of residents and medical/nursing students.


MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  1. Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
  2. National Certification by one of the recognized APRN Certification Centers.
  3. Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  4. BLS Certification required with additional Advanced Life Support training at the discretion of the department.
  5. Fundamentals of Critical Care Support (FCCS) Certification required immediately upon hire (based on class availability).

EXPERIENCE:

  1. Eligible new graduates will be considered.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  1. ACGNP certification or ACNP preferred, but would consider FNP.

EXPERIENCE:

  1. Acute care, critical care experience preferred.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.   1. Manages comprehensive and individualized care for patients and families in the Inpatient/Critical care phase of hospitalization.
  2. With the collaboration of Critical Care staff or by following established protocol and/or scope of practice and privileging guidelines, orders additional tests or consults to medically manage the patient.
  3. Performs, interprets, and communicates results of diagnostic tests.
  4. Consults and communicates with Critical Care and inter-professional services regarding abnormal findings in the evaluations of patients.
  5. Develops a comprehensive individualized plan of care for patients that include teaching, establishing disposition/discharge planning and/or necessary referral to other members of the health care team.
  6. Participates in physician/team rounds.
  7. Identifies unstable or complex patient care situations and facilitates appropriate intervention with appropriate resources.
  8. Recognizes emergency situations and initiates effective emergency care.
  9. Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation, and management of the individual needs of patients/families.
  10. Demonstrates the ability to utilize basic equipment in a safe manner.
  11. Facilitates patient discharge disposition or transfer by coordination of appropriate services.
  12. Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings.
  13. Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities, and readiness to learn.
  14. Updates own knowledge base throughout the year.
  15. Completes all identified clinical competencies.
  16. Mentors nursing personnel in all levels of clinical practice.
  17. Communicates effectively with patients/families, providers, interdisciplinary teams, and ancillary services at all levels to assure coordinated efficient patient care.
  18. Documents the plan of care, interventions, and evaluations in a clear and concise manner.
  19. Adheres to the established Performance Expectations for WVUM Employees in the areas of People, Service, Performance Improvement, and Shared Values & Culture.
  20. Performs initial assessment and develops plan for new admissions in conjunction with the critical care attending physician and/or team.
  21. Ensures patient summary (i.e., Problem List) and orders are current, comprehensive, and accurate.
  22. Ensures appropriate transition to next team when patient is ready to leave the ICU.
  23. Performs appropriate procedures as directed by Critical Care Team and/or within scope of practice.
