Elk Ridge Community Health is seeking an Advanced Practice Pharmacist to serve as the Pharmacist in our 340B Federally Qualified Health Center outpatient pharmacy located within Grand Canyon National Park. This role offers clinical autonomy and the opportunity to practice at the top of your license while serving a rural and underserved population within a Patient‑Centered Medical Home model. The position emphasizes collaboration with the care team and advancing patient safety and outcomes.

Qualifications

• PharmD or equivalent doctoral degree – Required.
• 3–5 years outpatient or retail pharmacy experience – Preferred.
• BLS certification required upon hire.

Responsibilities

• Provide comprehensive medication management including histories, reconciliation, adherence assessments, and education.
• Deliver evidence-based, patient-centered drug therapy recommendations and collaborate with the care team.
• Evaluate and verify medication orders for safety and appropriateness.
• Identify and resolve medication-related problems; support safety and quality initiatives.
• Serve as the primary clinical pharmacy resource for patients, providers, and staff.
• Apply 340B program knowledge to ensure compliance and access.
