Summary

Open continuous announcement for a Nurse Practitioner in a Patient Aligned Care Team (PACT) setting. The incumbent will provide medical care in an outpatient setting following PACT principles.

Responsibilities
  • Provide comprehensive primary care to a panel of assigned patients using the PACT model of care delivery.
  • Interface with non-VA providers (DoD and community) to ensure smooth transitions in care.
  • Contribute to achieving performance measure clinical outcome targets in the areas of access to care, quality of care, efficiency, process improvement, and patient satisfaction.
  • Prescribe medications and enter consults to specialty clinics using electronic medical records systems.
  • Practice in accordance with professional standards, core values, and a commitment to excellent service.
  • Complete clinical reminders for chronic disease management and prevention when due.
  • Document encounters, secure messages, and view alerts according to VHA timeliness standards.
  • Provide and coordinate clinical-related employee health services in conjunction with Human Resources.
  • Assist in completing Environmental Agent Registry exams and demonstrate proficiency in practice planning and self‑direction for each veteran’s medical needs, pursuant to the scope of practice for primary care (PA‑C).
  • Communicate with veterans, their representatives, visitors, and VA staff in a courteous, tactful, and respectful manner.
  • Provide consistent information to customers according to established policies and procedures.
  • Handle conflict and problems with customers constructively and appropriately.
  • Protect printed and electronic files containing sensitive data in accordance with the Privacy Act of 1974 and applicable laws, regulations, VA statutes, and VHA policy.
  • Use office automation software (word processing, electronic document retrieval, printer activation, text formatting, report generation, memos, and email transmission).
  • Use the Veterans Health Information and Technology Architecture (VistA) to access information in the Medical Center Computer System.
Qualifications
  • One full and unrestricted license from any U.S. state or territory.
  • Availability to work Monday–Friday, 7:30 am–6:00 pm (variable tours, compressed/flexible schedules allowed; telework not authorized).
