Job description

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. To qualify, applicants must meet specific eligibility requirements per VHA policy and submit their EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the application. Former EDRP participants are ineligible to apply.

Duties
  • The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers
  • This approach to healthcare empowers and equips people to take charge of their health and well-being and live their lives to the fullest
  • As an employee operating in a Whole Health System of care, you will work in a model with three core elements—seeking to create a personalized health plan for each Veteran
  • This is done in the context of healing relationships, healing environments, and a connection back to the Veteran's community
  • This aligns with the Veterans Health Administration (VHA) Mission Statement to honor America’s Veterans by providing exceptional health care that improves their health and well-being
  • The Advanced Practice Nurse (APN) or Certified Nurse Practitioner (NP) practices in Primary Care Services to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission
  • The NP provides advanced clinical practice, consultation, and management
  • Independently manages a panel of patients with acute and chronic health conditions, emphasizing health promotion and disease prevention using evidence‑based practice
  • Develops and implements individualized plans to achieve optimal population health and disease management
  • Demonstrates leadership and works collaboratively with the Primary Care Team and other interdisciplinary team members
  • Orders and interprets labs, tests, and diagnostic studies
  • Prescribes pharmacologic and non-pharmacologic interventions
  • Initiates referrals and consults to specialty services
  • Provides education to patients and their families
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

  • Providers may be required to travel to the CBOCs or provide gap coverage as determined by the supervisor
  • Provider must be a designated women's provider and complete mandatory VHA training
