Job summary

Tom Trese Networking LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization. This role operates within the Patient Centered Medical Home model and collaborates with physicians, ARNPs, PAs, and nursing staff to deliver high-quality care across a diverse patient population.

The ARNP will perform advanced clinical assessments, coordinate care, educate patients, and document accurately.

Qualifications

• Graduate of an accredited ARNP program
• Current RN and ARNP licensure in Washington state
• Prescriptive authority from Washington State
• Experience in broad primary care settings
• Willingness to participate in ongoing clinical education

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans to support patients' health
• Maintain accurate chart documentation within 48 hours, 95% of the time
• Meet productivity expectations (min 3,400 encounters/year)
• Provide care coordination through daily huddles in PCMH model
