Job description

Summary

The Advanced Practice Nurse position is located in the Specialty Care service at the Bedford VA campus. This is a Part-Time position, working 24 hours per week.

Duties

• Manage a panel of patients with acute and chronic health conditions, focusing on health promotion and disease prevention using evidence-based practices.
• Develop and implement individualized plans to improve population health and disease management.
• Collaborate with the Primary Care Team and interdisciplinary members, demonstrating leadership.
• Order and interpret diagnostic studies, prescribe pharmacologic and non-pharmacologic interventions.
• Initiate referrals to specialty services and provide patient and family education.
• Assess and adjust care plans to optimize patient outcomes.
• Manage patients with chronic pain, acting as the primary provider for pain management, especially for complex cases with co-morbidities, mental health diagnoses, and those on multi-modal therapy.
• Manage patients with Opioid Use Disorder and dual diagnoses, including prescribing buprenorphine.
• If not certified in addiction medicine, attend training within the first year.
• Coordinate pain management for patients referred to the interdisciplinary pain team.
• Lead the interdisciplinary team for patients with dual diagnoses, monitoring compliance and providing comprehensive care.
