Job Description
The advanced practice provider (Nurse Practitioner/Physician Assistant) within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care at the outpatient practices in Washington Township/Haddonfield NJ and surgical suites in Washington Township and Cherry Hill NJ.
Job/Practice Description
- Busy Musculoskeletal Care Focus: Evaluate and manage lower extremity, lower leg, ankle, and foot conditions.
- Schedule: Full-time, Monday - Friday, 7:00 AM - 3:00 PM
- Setting: Primarily office-based with some time at affiliated surgical centers
- Patient Volume: High volume of patients per day (ages 8 and above)
- Advanced Clinical Skills: Perform and/or assist with injections, including corticosteroids, biologics, and other therapeutic modalities.
- Office-Based Patient Triage: Function autonomously in an office setting, managing patient flow, pre- and post-operative evaluations, and comprehensive treatment planning.
- Surgical First Assist: Work collaboratively in the operating room, providing expert support in foot and ankle surgical procedures.
- Independent & Collaborative Functioning: Balance independent clinical decision-making with seamless team collaboration.
- Support our Foot & Ankle Surgeon in managing pre- and post-operative surgical patients.
- Conduct new patient evaluations and help build a pipeline of surgical cases.
- Perform in-office procedures, including injections, casting, splinting, pre-operative, intra-operative and post-operative care.
- Provide comprehensive care, including history and physical exams, and telehealth visits as appropriate.
- Utilize EPIC EMR for patient management.
- Access to ultrasound on-site.
Assessment & Management
- Performs complete history & physicals and documents in the medical record.
- Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
- The APP may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions.
- Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, systemic imbalance, immunocompetence, invasive interventions and diagnostic procedures.
Diagnosis
- Collaborates with interdisciplinary healthcare team in making diagnoses of acute and complex chronic conditions.
- Manages diagnostic tests through ordering, interpretation, performance, and supervision.
- Formulates differential diagnoses by priority.
- Diagnoses complications and orders appropriate interventions.
Formulates Plan of Care
- Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health needs.
- Utilizes evidence-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
- Implements and modifies the plan of care.
- Prescribes diagnostic strategies and therapeutic interventions.
- May perform advanced procedures consistent with privileges and competency validation.
Documentation
- Documents key components of patient's progress via daily progress notes, transfer, and discharge summaries, and/or clinic notes where applicable.
- Documentation is timely, meets compliance standards, and captures patient acuity.
Communication and Collaboration
- Maintains ongoing communication and collaboration with interdisciplinary healthcare team.
- Facilitates communication with patients, families, and staff to promote continuity of care across inpatient and outpatient settings.
Professional Practice
- Demonstrates professional behaviors including precepting, mentoring, education, and instruction of graduate students.
- Seeks opportunities for research and evidence-based practice analysis.
- Participates in Grand Rounds and specialty meetings.
- Maintains CEUs and membership in professional organizations.
Qualifications
- Master's Degree
- National Board Certification as a Primary/Family Care Nurse Practitioner or Physician Assistant
- BLS certification
- Extensive experience in musculoskeletal pathology, especially lower extremity, ankle, and foot conditions.
- Proficiency in injections (corticosteroids, biologics, viscosupplements, etc.) preferred.
- Surgical first-assist experience or training in foot & ankle surgery is highly desirable.
- Strong ability to triage, diagnose, and manage patient care autonomously.
- A passion for surgical collaboration and growth in a high-volume practice.
