Job summary

Piedmont Healthcare is recruiting an Advanced Practice Practitioner to join the Primary Care team at our outpatient practice in Sweetwater. The position is full time, Monday through Friday, with responsibilities across direct patient care and collaborative physician support.

The APP will focus on health promotion and chronic disease management, working with multiple IM providers to serve the PCP patient panel in an outpatient setting.

Qualifications

• Minimum 2 years of experience as Physician Assistant or Nurse Practitioner.
• Master's or Bachelor's degree from an accredited PA program and national board certification.
• Licensed as a Nurse Practitioner in Georgia with NP credential.

Responsibilities

• Provide medical services to individuals, families and/or groups.
• Emphasize health promotion and management of acute and chronic disease.
• Augment a physician's ability to provide medical services to patients.
• Document data, perform diagnostic and therapeutic procedures, order labs and consultations.
• Collaborate with multiple IM providers to care for the PCP patient panel.
