Sutter Health's PAMF-Palo Alto Medical Foundation CAD is seeking a Nurse Practitioner to assess, diagnose, plan and implement patient care in collaboration with physicians. You will deliver holistic, patient-centered care across ambulatory and inpatient settings, adhering to CA Nurse Practice Act and safety standards.

Graduate of an accredited nursing program with CA licensure is required; 5 years of relevant experience is typical.

Qualifications

• Graduate of an accredited school of nursing.
• Master’s: Nursing

Responsabilités

• Manages the medical care for patients utilizing clinical protocols in collaboration with the physicians.
• Performs a physical exam if indicated.
• Ensures individualized health maintenance status to include vaccinations and risk factor screening.
• Consults with supervising physician in cases of abnormal findings and when implementing clinical protocols.
• May facilitate care to a community practitioner, upon discharge.
• Conducts patient and family assessment encompassing physiological, spiritual and emotional areas.
• Collaborates and communicates with physicians and other team members to interpret and coordinate care.
