Job summary

Schoolcraft Memorial Hospital in Michigan seeks an Advanced Practice Provider to deliver direct patient care within a rural health clinic setting. You will evaluate, treat, and manage a diverse patient population, collaborating with physicians to ensure high-quality outcomes.

Requirements include graduation from an accredited APP program, board certification (NP/PA), MI licensure, DEA, and BLS/ACLS/PALS certifications.

Qualifications

• Graduate of an accredited advanced practice practitioner program.
• Board certified as NP or PA with Michigan license.
• Current DEA license and BLS, ACLS, and PALS certifications.

Responsibilities

• Participates in development, execution and periodic review of clinic policies.
• Provides services per policy and refers patients for needed services not available at the clinic.
• Maintains patient health records and transfers as required when patients are referred.
• Participates with a physician in periodic review of patient health records.
• Provides acute non-emergency care to walk-in patients.
• Consults with family physicians regarding patient care as necessary.
• Dictates or provides a progress note on all visits and calls.
• Maintains patient confidentiality and complies with RHC standards.
• Ensure follow-up on abnormal test and x-ray results.
• Perform other duties as assigned.
