Pride Health is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to support its Senior Community Care program in Maryland. Candidates must hold an active NP or PA license and a Master's degree, while experience in long-term care and strong communication skills are preferred. The position covers Wicomico & Accomack counties and includes travel during work shifts.

Qualifications

• Active and unrestricted state NP or PA license, or ability to obtain by start date.
• Master’s degree or higher (NP or PA).
• National board certification (AANP, ANCC, or NCCPA).

Responsibilities

• Conduct comprehensive patient assessments.
• Manage complex acute and chronic medical & behavioral health conditions.
• Collaborate closely with the interdisciplinary care team.

Skills

1+ year of clinical experience

Strong communication skills

Ability to lift a 30-lb clinical bag
