Job summary

Atrius Health, part of the Optum family, seeks a Nurse Practitioner or Physician Assistant to join our Chelmsford, MA team. You’ll provide longitudinal primary care, coordinate care for a patient panel and supervise care plans in collaboration with specialists.

Qualifications

• Master's degree in Nursing (NP) required or preferred.
• Licensed by the MA board of registration in Nursing.
• National Board Certification.
• Mass DPH CS registration or ability to obtain post-hire.
• DEA licensure or ability to obtain post-hire.
• BLS certification in good standing.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of a patient panel.
• Examination, diagnosis and treatment in primary care; supervise care plan and referrals.
• Monitor and manage acute and chronic illnesses; act as care plan author and supervisor.
• Partner with patients and families to manage care plans.
• Provide consultation for hospitalized patients in the panel.
• Provide direction and support to staff to ensure department effectiveness.
