Qualifications

• Master’s degree in Nurse Practitioner or PA, or comparable education/experience.
• Current Arizona Nurse Practitioner license or Arizona Physician Assistant certification (PA-C).
• Comfortable initiating goals-of-care and end‑of‑life discussions.
