Job summary

Clare Medical of New Jersey is seeking an APN to perform comprehensive patient assessments, wellness visits, and management of acute and chronic diseases in a collaborative primary care setting.

The role requires NJ APN licensure, DEA/CDS, and the ability to travel within the region; Medicare experience and bilingualism are a plus. Excellent salary with bonus opportunities and comprehensive support are offered.

Qualifications

• Unrestricted APN license in NJ with at least 1 year clinical experience.
• Current DEA/CDS license required.
• Willingness and ability to work independently.
• Ability to travel within the region.
• Bi-lingual a plus; Medicare experience a plus.

PRIMARY RESPONSIBILITIES:

• Initiate and complete detailed patient assessment & physical exams
• Wellness exams and sick visits
• Evaluate and manage common and acute chronic diseases
• Order, interpret and evaluate diagnostic and lab results
• Refer and collaborate with other providers
• Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

• Computer literate & EMR Proficiency; Experience with Athena a plus
• Unrestricted APN license in the State of NJ with at least 1 year clinical experience
• Current DEA/CDS
• Willingness and ability to work independently
• Personable, passionate and energetic
• Ability to travel within the region
• Previous experience with Medicare eligible patients a plus.
• Bi-lingual a plus
