Clare Medical of New Jersey is seeking an Advanced Nurse Practitioner to provide in-home physician services across Ocean County, NJ. The role focuses on detailed patient assessments, wellness and sick visits, and comprehensive disease management.

Qualifications

• Unrestricted APN license in New Jersey with at least 1 year of clinical experience.
• Current DEA/CDS clearance.

• Computer literate with EMR proficiency.
• Previous Medicare patient experience a plus.

PRIMARY RESPONSIBILITIES:

Initiate and complete detailed in home patient assessment & physical exams

Wellness exams and sick visits

Evaluate and manage common and acute chronic diseases

Order, interpret and evaluate diagnostic and lab results

Refer and collaborate with other providers

Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

Computer literate & EMR Proficiency

Unrestricted APN license in the State of NJ with at least 1 year clinical experience

Current DEA/CDS

Willingness and ability to work independently

Personable, passionate and energetic

Ability to travel within the region

Previous experience with Medicare eligible patients a plus.
