Job summary

The University of California, San Francisco (UCSF) Heart & Vascular Center is seeking a full-time Advanced Practice Provider to care for patients with advanced heart failure, including MCS and transplant, in inpatient and multidisciplinary settings.

As a primary provider in the hospital, the APP will collaborate with cardiology, surgery, ICU teams and care coordinators to evaluate, treat, and manage complex patients across the continuum of care, including discharge planning and follow-up.

Qualifications

• Graduation from a California Board of Registered Nursing approved or verified school of nursing with a master’s degree in nursing or a Doctor of Nursing Practice (DNP).
• Minimum of 4 years as a Nurse Practitioner in Cardiology, MCS, or Advanced Heart Failure.
• Minimum of 2 years managing mechanical circulatory support patients and 3 years critical care nursing experience with MCS devices.

Responsibilities

• 60% Inpatient - Direct Patient Care: evaluate and manage patients, document in EHR, order medications and tests, participate in rounds and discharge planning.
• 25% Inpatient – Care Coordination: coordinate hospital admission, discharge planning, follow-up scheduling, and cross-disciplinary communication.
• 10% Outpatient – Independent Encounters: manage patients via in-person and telehealth encounters with appropriate documentation.
