Job summary

University of Minnesota Physicians seeks a Certified Nurse Practitioner or Certified Physician Assistant to deliver expert care on the Heart Care - Heart Failure team at the University of Minnesota Medical Center and CSC in Minneapolis.

You will join a multidisciplinary group, provide follow-up in clinics, participate in inpatient rounds, and coordinate tests and patient education, using EPIC in a collaborative hospital environment.

Qualifications

• Graduate of an accredited Physician Assistant or Adult Nurse Practitioner training program.
• Eligible or certified through NCCPA, ANCC or AANP.
• Licensed to practice with MN Board of Medical Practice (PAs) or MN APRN license, or eligible for MN Board of Nursing.
• Able to meet credentialing and privileges requirements.

Responsibilities

• Deliver care for adults with Heart Failure including transplant and MCS patients.
• Perform independent follow-up in clinic.
• Participate in inpatient rounds and develop plans of care.
• Order tests, interpret results, and coordinate follow-up.
• Communicate effectively with patients, families, and hospital staff.
