Methodist Hospital, part of Methodist Healthcare, seeks an Advanced Clinical Pharmacist to contribute to bedside rounds, drug utilization reviews, and development of clinical services. You will mentor staff and collaborate with multidisciplinary teams to ensure accurate documentation and optimal patient care.

The role requires PharmD or BS in Pharmacy, hospital pharmacy experience, PGY1 or residency, BCPS certification, Texas pharmacist license, and ATT if licensed in another state, with

Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
• Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required; preferably within area of focus
• Completion of hospital PGY1 / pharmacy general practice residency or fellowship
• BCPS certification or other Board of Pharmacy Specialty (BPS) certification
• Current Registered Pharmacist licensed by the Texas State Board of Pharmacy
• Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy.

Responsabilités

• Interpret orders and transcribe or verify them accurately into the pharmacy computer system
• Accurately compound and dispense pharmaceuticals
• Comply with facility policy and procedures and regulatory requirements
• Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
• Provide clinical consultation and clarification to practitioners; suggest cost-effective therapeutic alternatives
• Sustain the hospital formulary and promote rational drug therapy selection
• Attend and actively participate in daily multidisciplinary and bedside rounds
• Support formulary decision processes with medical staff to promote rational therapy
• Provide accurate and timely medication information to healthcare staff and patients
• Participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs.
