Job summary

The Womans Hospital of Texas is recruiting an Advanced Clinical Pharmacist to lead medication-related policies and service lines within a multidisciplinary hospital team. You will advance pharmacy practice, educate pharmacy and interdisciplinary teams, and mentor residents while upholding safety and ethics in patient care.

You will also oversee CPOE accuracy, drug therapy management, TPN/chemotherapy preparation, and adherence to USP 797/800 guidelines, ensuring safe and cost-effective therapies

Responsibilities

• Leads committees responsible for establishing and implementing medication-related policies and procedures (e.g. Pharmacy & Therapeutics, Medication Safety, Antimicrobial Management Program)
• Directs one or more pharmacy service lines (e.g. cardiovascular, pediatrics, critical care) within the hospital
• Demonstrates leadership through measurable improvements in pharmacy practice
• Provides regular, formal education to pharmacy and interdisciplinary teams
• Mentors students, residents, and colleagues to deliver high-quality pharmaceutical care
• Verifies CPOE orders for accuracy and intervenes when orders are unclear or unsafe
• Interprets medication orders, updates patient profiles, and documents in medical records
• Prepares and dispenses medications including sterile, chemotherapy, and parenteral nutrition safely following USP guidelines
• Evaluates TPN appropriateness using facility or ASPEN guidelines
• Manages controlled substances with proper records and compliance
• Ensures evidence-based, cost-effective medication therapies across patient care
• Monitors therapy for interactions, allergies, and dose appropriateness
• Develops and implements drug therapy management protocols (pharmacokinetics, anticoagulation, IV to PO, pain, antimicrobial stewardship)
• Reviews culture and susceptibility data to guide antibiotic prescribing and duration of therapy
• Reads and interprets patient charts, detects adverse reactions and medication errors, and escalates as needed
