You will collaborate with the Transitional Coordinator and Adult Services Coordinator, educators, families, and community partners to manage administrative tasks related to transition plans and connect students to real-world learning opportunities.

Qualifications

• Experience supporting ISPs/IEPs is preferred.
• Strong organizational and communication skills required.
• Ability to collaborate with families, educators and coordinators.

Responsibilities

• Support students with ISPs and IEPs as they prepare for life beyond school.
• Coordinate with Transitional and Adult Services Coordinators and staff.
• Handle administrative tasks related to transition plans and learning opportunities.
