Job summary

Southwestern Behavioral Healthcare in Indiana is seeking a State Psychiatric Hospital Liaison to coordinate transitions for adults with serious mental illness. You will link SPH settings with Community Treatment Facilities, arranging patient transfers and sharing progress updates.

The role requires a Bachelor's degree in a mental health or human services field, at least two years of relevant experience, and a valid Indiana driver's license; crisis management training is a plus.

Qualifications

• Bachelor's degree in mental health or human services field required.
• Minimum two years of experience working with long-term psychiatric patients required.
• Must be at least 21 years old with valid Indiana driver's license and car insurance.
• Crisis management and verbal de-escalation training is required or must be obtained.

Responsibilities

• Provide school and community-based case management and skills training to children and families.
• Coordinate referrals between School, Department of Child Services, Juvenile Probation, System of Care, Southwestern assessment team and therapists.
• Maintain records reflecting patient progress and update treatment plans and goals.
• Submit reports within established time frames and maintain documentation.
• Communicate professionally and coordinate services with community agencies.
