Qualifications

• Bachelor's degree in Human Services or related field and a valid driver’s license with acceptable driving record required.
• Reference/background screening for all positions is required.
• One year of mental health related work experience preferred.
• LAC or LMAC preferred. Education/Experience substitutions are possible.

Responsibilities

• Conduct assessments of individuals to identify needs and determine service eligibility; communicate information to individuals, families, and stakeholders.
• Develop plans to address needs and coordinate resources with community agencies.
• Provide a designated caseload level of services in a community-based setting; organize skill development and support groups.
• Complete and submit documentation; evaluate progress and measure outcomes.
