Job summary

Johnson County Government's Mental Health division is seeking a Case Manager for older adults (65+) with severe mental illness. The role combines community-based and office-based services, using strengths-based practices to connect clients with needed resources and supports.

You will conduct assessments, develop care plans, coordinate with agencies, and monitor progress under measurable goals while ensuring licensing and screening requirements are met.

Qualifications

• Bachelor’s degree required in Human Services or related field.
• Valid driver’s license with acceptable driving record is required.
• Johnson County Government requires reference/background screening for all positions.
• Education/Experience substitutions allowed (experience may substitute for education and vice versa).

Responsibilities

• Conduct assessments to identify individuals' needs and determine service eligibility; interpret and communicate information to individuals and families.
• Develop plans to address needs and coordinate with community agencies to research and arrange resources.
• Provide services for a designated caseload in community and office settings, including transportation as needed.
• Complete and submit documentation; evaluate progress toward measurable goals and review service effectiveness.

Johnson County Mental Health is looking for a qualified candidate to fill the position of Case Manager for older adults on the Older Adult Team. This position works with adults ages 65+ with severe mental illness. This team utilizes principles of Strengths-Based Case Management, providing a range of mental health services in both the community and office-based settings with adults experiencing a severe mental illness. First review of applications will be September 25.

Job Description

Job Duties:

• Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders.
• Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long term individual needs; encourages the establishment and maintenance of support systems; consults and collaborates with other providers in the development, implementation and review of plans.
• Provides a variety of services for a designated case load of individuals with multi-faceted and/or ongoing needs; ensures services provided are consistent with individual plans; initiates and attends individual and/or group meetings and provides services in a community based setting; provides transportation of individuals according to the needs identified in plans; organizes and facilitates skill development and support groups.
• Completes and submits documentation regarding services provided; evaluates and documents individuals' progress according to measurable goals; reviews quality and effectiveness of services provided.

Job Requirements

Minimum Job Requirements:

Bachelor’s degree in Human Services or related field and a validdriver’s license with acceptable driving record required.

Johnson County Government requires reference/background screening for all positions. Specified criteria may vary by Department/Agency.

Preferred Job Requirements:

One year of related work experience is preferred.

Education/Experience Substitutions:

Experience may be substituted for degree. Education may be substituted for experience.

In some instances, statutory or regulatory requirements may prohibit substitution of education and/or experience.

All County employees may be called upon to assist other departments in a declared emergency situation.
