Job summary

Ability KC in Kansas City is seeking a Case Manager for Adult Medical Rehabilitation and ACCT. You will monitor resource use, support efficient discharge planning, and obtain necessary certifications for rehab stays.

You will ensure compliance with regulatory agencies and work as part of a multidisciplinary team to deliver high-quality care. The role emphasizes advocacy for people with disabilities, excellent interpersonal skills, and coordination with insurers, physicians, and service providers

Qualifications

• Knowledge of medical rehabilitation and disability services helpful.
• CPR certification or eligibility is required or preferred.

Job description

Under the general direction of the Adult Medical Rehabilitation and Adaptive Computer & Communication Technology (ACCT) supervisors, responsibilities include monitoring appropriate utilization of program resources, efficient discharge planning, obtaining certifications for rehab stays, ensuring compliance with rules set forth by regulatory agencies, and performing other duties as assigned. The case manager will work as a member of a multidisciplinary team.

Education

• High school diploma or above

Experience

• A minimum of two years working in a role that supported people with disabilities and advocating for their needs.
• Experience working in a medical rehabilitation facility preferred.
• Experience working with insurance companies, physician offices, and Medicaid and Medicare preferred.

Knowledge/Skills/Licensure/Certification

• Knowledge of the medical, school and support system people with various disabilities access.
• Is able to advocate for persons served with public and private agencies, and insurance industry representatives.
• Excellent interpersonal skills required.
• CPR certification or eligible.
