Qualifications

• Master’s degree earned from an accredited college or university in relevant human services field.
• LMHP eligibility required; license preferred.
• Experience or relevant course work with completing assessments and providing basic counseling interventions required.
• Must possess a valid Driver’s License, good driving record, and the ability to provide your own transportation.

Responsibilities

• Provide initial intake sessions for new clients via the SDA Walk-in Clinic and short-term crisis intervention counseling.
• Gather psychosocial and treatment history on new clients and provide referrals to community resources.
• Complete case management eligibility and co-occurring assessments for clients.
• Conduct SDA follow-up appointments to monitor treatment engagement.
• Coordinate referrals with supervisory staff to ensure continuity of care.
• Submit documentation timely and manage releases of information.
• Conduct detox/rehab assessments as directed.
• Support development of new programs as needed.
• Complete Intake Assessments for all service lines and coordinate follow-up care.
• Conduct individual therapy as directed.
• Perform other duties as assigned by supervisor.
