Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge needs.
• Formulate and revise the plan of care in collaboration with the multidisciplinary team.
• Implement care per physician orders and nursing plan; supervise other staff as needed.
• Evaluate patient progress and adjust care; document findings and interventions.
• Promote continuity of care through clear communication with the care team and families.
