Job summary

Harris County is seeking a Guardian Case Manager to oversee and coordinate case management services for incapacitated adults declared wards. You will manage ward care plans across legal, medical, financial, social, and psychiatric aspects, ensuring tailored care and adherence to regulatory requirements.

You will establish living arrangements, coordinate treatments, and maintain meticulous case documentation. The role includes ongoing ward visits, financial duties, and post-mortem tasks as needed.

Qualifications

• Bachelor’s Degree in a Social Science field
• At least 1 year of case management experience with adults in a social service setting
• Proficiency with Microsoft Office and case management software; valid Texas driver’s license; ability to pass background check and required certifications

Responsibilities

• Develops, implements, and manages ward care plans tailored to each ward’s needs and functioning level
• Establishes and monitors living arrangements in the ward’s best interest
• Coordinates medical and psychiatric treatments for wards
• Maintains comprehensive, timely case documentation (petitions, court reports, correspondence, notes)
• Applies for public and private financial and medical benefits
• Fulfills financial powers and duties of a Guardian and completes required reports
• Provides ongoing case management with monthly ward visits and residence inspections
• Facilitates funeral arrangements and notifies all parties upon a ward’s death
