Job summary

Advanced Healthcare Integration seeks a Nurse Practitioner to provide in-home patient care after hospital discharge, traveling to patients in Brevard County. The goal is to reduce readmission and learn our backend business processes for patient care and back-office protocol.

The role includes on-call weekend coverage and flexible scheduling as a 1099 independent contractor. Must carry your own liability policy and be fully licensed APRN, with autonomous status preferred.

Job description

We are an in-home medical group looking for a Nurse Practitioner to see patients post-hospital discharge with the goal of reducing readmission. Patients are based in Brevard County. This role involves travel to each patient location.

This role also includes business management as well. We are looking for a nurse practitioner who also wants the opportunity to learn our business on the backend to manage patient care and back office protocol. This role does include on call for some weekends each month.

Responsibilities

• Televisit for follow-up care as needed.

Other Details

• A full-time schedule is ideal, however, there is flexibility in creating your schedule.
• Compensation is based on the reimbursement for each patient that you conduct a home visit with.
• This is a 1099 independent contractor role.
• Must carry your own liability policy.
• Must be fully licensed APRN, autonomous status preferred.
