Job summary

PEOPLE'S HEALTH CENTERS is seeking an Adult Community Support Specialist in St. Louis, MO. This full-time role supports individuals at risk of hospitalization or recently discharged to live successfully in the community.

The CSS conducts assessments, helps develop person-centered plans, monitors progress, and teaches living and social skills. It also transports clients to appointments and maintains records in line with clinical guidelines.

Qualifications

• Bachelor's degree and two years of qualifying experience.
• Any four-year combination of higher education and qualifying experience.
• An individual with four years of qualifying experience.
• Qualifying experience must include delivery of service to individuals with mental illness, substance abuse or developmental disabilities.
• Experience must include some combination of the following: one-on-one or group services, teaching coping skills, and supporting recovery.

Responsibilities

• Provide activities and interactions that help at-risk individuals remain in the community after discharge.
• Evaluate client status and participate in person-centered planning and goal attainment.
• Train clients in living and social skills and pro-social activities to support recovery.
• Transport clients to appointments and conduct house visits as applicable.
• Maintain client records and adhere to Clinical Treatment Team guidelines.
