This role involves supporting high-risk behavioral health patients aged 18-64 post-discharge, conducting follow-up calls, and facilitating transition to community services. It is a full-time, hybrid position based at 399 Revolution Drive, Somerville, MA, with a schedule of Monday-Friday, 8:30 am-5:00 pm, offering some flexibility.

Qualifications:

• Current Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW).
