Job summary

Saint Francis Ministries is seeking a Case Manager for Adoption to provide comprehensive in‑home case management and permanency services to children, youth, and families throughout Kansas. You will assess safety, develop and implement case plans, coordinate services, and support adoptive placements with court documentation and ongoing communication with families.

The role requires travel across the state, 21+ age, a valid driver’s license, and background clearances.

Qualifications

• Bachelor’s degree in social work or human service field required.
• Two years’ experience with children/youth and families preferred.
• Must be 21 years of age and able to pass background checks.

Responsibilities

• Provide direct in-home case management and permanency services.
• Assess safety and complete safety plans for children/youth and families.
• Travel across Kansas to deliver services and attend court when required.
• Develop and review case plans to achieve permanency goals within time frames.
• Coordinate services including mental health, drug/alcohol, respite care, and supervised/unsupervised visits.
• Prepare families for moves, grief and loss issues related to adoption.
• Maintain accurate documentation and reports for court and case records.
