Job summary

Saint Francis Ministries is seeking a Case Manager for Adoption to provide comprehensive case management and permanency services to children, youth, and families navigating the adoption process. You will perform in-home services, safety assessments, and ongoing planning, while coordinating with courts and community partners to achieve permanency goals.

The role requires travel across Kansas, adherence to policy, and strong documentation practices to support finalization and post-finalization

Qualifications

• Bachelor’s degree in social work or human service field from an accredited college.
• License to practice in the State of Kansas preferred.
• Prefer two years’ experience working with children/youth and families.
• Must be 21 years of age and able to pass required background checks.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provides direct in-home case management and permanency services.
• Assesses safety and develops safety plans for children/youth and families.
• Travel throughout Kansas to deliver services and support.
• Documents case plans, reports, and court-related materials accurately.
• Collaborates with courts, agencies, and families, including potential testimony.
• On-call rotation to address urgent issues after hours.
