Job summary

Mindoula Health's Collaborative Care Program is expanding to Oregon, aiming to embed behavioral health into primary care to improve outcomes.

Clinicians will work in a hybrid model, with Eugene, OR clinics and remote support. A Master's degree in Social Work, Counseling, Psychology, or related field, plus CSWA and adolescent experience, EMR/telehealth familiarity, are required.

The role emphasizes supervision, weekly case consultation, and meaningful autonomy within outpatient hours.

Qualifications

• Master's degree required in social work, counseling, psychology, or related field.
• Experience with adolescent populations.
• Experience with EMRs and telehealth workflows.

Responsibilities

• Screen and assess for mental health and substance use disorders.
• Provide short-term, evidence-based interventions (BA, PST, MI).
• Support medication adherence and monitor side effects.
• Provide education and self-management support.
• Track clinical response via in-person, video, or phone and adjust plan.
• Refer to specialty care when appropriate.
• Participate in weekly psychiatric case consultation.
• Document in the EHR per model workflows.
• Re-engage patients who fall out of care.
• Practice under supervision of independently licensed clinicians.
