Qualifications

• Bachelor's degree in Social Work or psychology from an accredited college.
• Demonstrated experience with behavioral health services.
• Must be at least 18 years of age.
• CPR, CPI and First Aid certifications required on hire or within 30 days.
• Complete post-offer physical, background checks and stated verifications.

Responsibilities

• Identifies and plans for psychosocial needs of members served.
• Designs, implements, and evaluates Recovery Plans with the treatment team.
• Provides education to members and families about mental health options.
• Discharge planning including referrals and coordination with the Business Office.
• Maintains awareness of community resources and rights tracking.
• Provides consultation regarding admission, discharge, and aftercare plans.
