Job summary

University Hospitals is seeking a full-time Licensed Professional Counselor to support adolescent patients within the Behavioral Health – Adolescent IOP Program in Cleveland. The LPC will contribute to patient care in a collaborative, interdisciplinary team.

The role emphasizes psychosocial assessment, discharge planning, and linking patients with community resources to ensure optimal outcomes for youths and families.

Qualifications

Licensed Professional Counselor with Ohio licensure (LPC).
Experience providing psychosocial assessments and interventions.
Ability to collaborate with multidisciplinary teams and link to community resources.

Responsibilities

Provides psychosocial assessment / intervention for adolescents and families.
Collaborates with interdisciplinary team to coordinate care.
Links patients to community resources to facilitate optimal care.
Delivers age-appropriate care across the adolescent age range.
Develops and implements comprehensive psychosocial/discharge plans.
Maintains knowledge of UH network services and promotes their use.
Provides guidance to staff on complex psychosocial and discharge planning issues.
