Vaiden Community Living Center is seeking an Admissions/Social Services Director to grow the census through referrals, outreach, marketing initiatives, and efficient admissions coordination. You will oversee the social services program to support residents’ physical, mental, and psychosocial well-being in line with regulations.

Responsabilidades

• Build and maintain relationships with referral sources and follow up regularly.
• Meet census goals and maintain Medicare/private-pay census.
• Coordinate admissions from inquiry to placement and oversee pre-admission assessments.
• Oversee social services programs and ensure resident well-being per policies and regulations.
• Collaborate with administrators and nursing leadership on clinical and financial considerations.
• Ensure a welcoming transition for new residents and families.
• Maintain social services documentation and regulatory compliance.
