We are hiring for a Hospice Admissions RN!

Responsibilities

The Admissions Nurse is responsible for executing the hospice admission process in compliance with federal and state regulations. The nurse will advocate for the patient and family to meet their needs that hospice can cover or refer them to other resources as appropriate.

• Demonstrate competency in assessing potential patients, determining hospice eligibility, and meeting with families to assess their needs.
• Maintain confidentiality and safety of patient/client clinical records.
• Complete assigned admissions and patient care orders effectively and timely.
• Communicate effectively with physicians, referral sources, patients, families, and the hospice interdisciplinary team regarding new admissions and relevant records for hospice eligibility.
• Coordinate with operational, clinical, and sales leadership regarding the status of pending referrals.

Education and Experience

• Must be a licensed registered nurse in the state of operation.
• At least 2 years of experience in hospice, home health, or another healthcare service delivery setting.
• Current CPR certification.
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• In Louisiana: a minimum of two years full-time experience as an RN. Alternatively, two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for RN experience.
